Liver enzymes

Started MTX 8 weeks ago for PsA. I am on 10 mg a week with one 5mg folic acid. Pre MTX LFT was 38 and reading at 4 weeks post MTX has risen to 40. Rheumatologist said my liver enzymes were high at 38 and higher still at 40 and warned me to lose weight and not touch a drop of alcohol. I am trying so hard but I am making myself miserable by being afraid to have the odd treat like a glass of wine, a meal out or a bit of chocolate. When I google liver enzymes all the websites say that my readings are nothing to worry about. Any advice please?

Oh and I forgot to say I am overweight but nowhere near obese. I am a size 14 in UK sizes

Sorry Linda, but the only advice I can give is follow exactly what your rheumatologist has advised. 10 years ago I followed a strict low carb eating plan involving medically prescribed shakes, and lost 20-25 kg. Many people I met were very enthusiastic to hear more about it, but as soon as they heard the first 2-3 months required ZERO alcohol and zero treats like occasional chocolate they lost interest and said they were not prepared to do such a thing. You might not be obese, but overweight is overweight. I WOULD suggest that at your next visit to rhe rheumatologist, you ask to discuss your enzyme levels in relation to what you have found on "websites". Maybe your rheumatologist is concerned about the rising trend in your levels and not only the absolute number? You should be able to have that discussion, but of course not framed in terms of "I think you are wrong because I read xxx on the internet" :-)) With all respect, if you were a close friend of mine, I would suggest that you think about your health objectives vs wanting to find strangers to validate your desire to have the odd glass of wine and other treats. Can you not have a meal out that embraces the social occasion but does not involve alcohol or a calorie-laden blow-out?

Thanks for your reply 'OhNo_NotAgain'. Your response is a stark reminder of why I was so dubious about posting on here. Why are you so judgemental? Who indeed do you think you are? Do you think I am stupid? What fool in their right mind would say to a health professional 'I think you are wrong because I read xxx on the internet?' I am extremely GLAD that you are no friend of mine, you sound like the type of sanctimonious oik who loves lording it over others because you have lost your fat!!!!!! Jog on 'shake boy'!!

Hi Linda - Many years ago I was on MTX and after 1 month or perhaps two the dermatologist rang me to say my liver had a problem and I must come in immediately. When I went they were convinced I was drinking a lot and made me say in bottles rather than units in case I was too dumb to give the right answers. I don't think they ever told me the numbers, or I don't remember them, but I had to come off it - so it clearly doesn't suit everyone. I wasn't told NOT to drink, though, just not to drink more than a couple of glasses as week,

LindaH, I apologisde for not telling you that it is fine to have your wine and to ignore your rheumatologist who is concerned about your liver, and seemingly your overall weight (which an also have adverse effects on your liver, amongst other things). Clearly your rheumatologist is wrong and does not understand your condition like "google" does. Obviously it is of no concern that your enzyme levels are not stable but are increasing. I do hope that you have friends who will only agree with you and support your point of view to validate you, no matter the affect on your health. After all who needs friends to be concerned about one's health or to be truthful ? Oh, and by the way, I am not a "boy". Best wishes.

Odd! Who said anything about ignoring the specialist? Who suggested the rheumatologist was wrong? Your passive, aggressive reply really does make me giggle. You have no idea what I have asked clearly!!!

Psoriasis 40 years, most treatments tried over this time, currently on biologics (stelara) as nothing else work.

Can I add that MTX can cause liver damage as per contraindications. We are all different, some will tolerate well, others not so well. We all know what we should be doing, but we are not perfect (those that think they are.....well!). You need to do what's right for you and what keeps you 'happy'. Psoriasis, and I believe Psa sufferers, have higher tendencies to anxiety, depression, non-alcoholic fatty liver disease, heart disease, obesity etc. It's a fine balancing act. Some of us have adverse reactions to different medications that do not make sense. This is why they do so many blood tests, to find who is suitable for certain drugs or not. I know someone who drank everyday on MTX and was fine, bit others not so. We are unique and we have different life challenges. Be kind. Good luck with your treatments.

Thanks for your post Sizzy. I 100 percent agree that we need to be kind. If people are brave enough to come on here and ask for advice then I think others ought to give kind advice and if they cannot do that then it is best to just not post anything. I mean you can disagree with people but there are polite ways to disagree rather than stooping as low as using rude, insulting insinuations. I hope you are feeling well and that the Stelara is helping. 🥰
